Dovecot uses maildir++, which the maildir patch understands; all you have
to do is to configure Alpine to read a maildir.

I agree. Dovecot knows more about its folders than Alpine will ever know.
I recommend to use dovecot for exclusive access in this case. However, I
would consider to eliminate dovecot if it is not needed. If the purpose is
reading e-mail with Alpine, the maildir patch is good enough for this
purpose. In other words, one can solve the problem
MDA + Dovecot + Alpine
by using
MDA + Maildir Patch for Alpine
