Support Group Paperwork

Hello All,  Does anybody have the time and inclination to lead a rookie
like me through the maze of paperwork required to set up an Amputee
Support Group??  I am talking about Form SS-4 (Application for Employer
Identification Number), Form 1023 (Application for Recognition of
Exemption), Form 8718 (User Fee for Exempt Organization Determination
Letter Request), these are just the Federal forms.  I also need help
with the State (Texas) forms for Incorporation and Franchise and
probably others that I don't even know about.  I did register our group
name with the County and have some of the forms (federal) on hand. Any
help would be appreciated and I would guess that others would be able to
