Here's a website that fits that description, don't know about a document.

On another topic, I remember reading on this listserve that is now a completed document that lists the accepted abbreviation/acronyms for medical terms. Where can I get a copy of this?

Friends:

What to do with two line URLs (Uniform Resource Locators or Website
addresses) can be a problem. When you see a URL that spans two lines it's
because it's longer than 80 characters, the break-off point for most email
readers. If you're working with a Web browser only half the line will get
URLed so that when you click on it you'll get a 404 or similar error.

Here's what to do. Cut and paste the first line into the Location bar on
your browser - or click on it (and don't worry about the error message).
Then cut and paste the second line into the URL after the first (broken)
part being careful not to delete the first part of the URL. Check to make
sure you got all characters.

Hit enter and go.
