elderly grant proposal

I am currently a dietetics student at CSU, Fresno doing a grant proposal 
assignment for an upper division nutrition class.  My topic is to 
formulate a community nutrition program for low-weight low-income 
elderly.  I am in need of information on successful and unsuccessful 
programs that have been established in the past.  The only program I have been able to find so 
far is "Meals on Wheels."  Email addresses of reliable sources would be 
very helpful, if possible.
