Help locating authors

Hope some of you RDs out in California can help. I'm trying to start a DM ed
program for Hispanics here in Colorado, and have located a booklet dated
1991, called "Comer Bien Para Vivir Mejor" or "Eat well to live better."
It's a skills-oriented nutrition counseling help for Mexican American Type
II Diabetic Patients. Authors are: 
        Teri A. Hall, MPH, RD
        Karen Bertram, MPH, RD
        Susan B. Foerster, MPH, RD
in cooperation with the Diabetes Control Program, State of California. Its
production and distribution was supported by The Upjohn Company.
Unfortunately, there's no address listed, so I can't write to find out how I
can get a copy. I'm hoping some public health nutritionist has either heard
of the book, or knows one of the authors, and will post the address so I can
write or call for more information.
