This will not help you right now, but later this year (perhaps as 
soon as May ADA could better tell you) a book called, Medical 
Nutrition Therapy Across the Continuum of Care, is coming out of ADA 
publications.  I have seen part of it and it has, for example, 
Medical Nutrition Therapy Protocol for Hyperlipidemia, Ambulatory 
Nutrition Assessment forms,  etc.  It is going to be very 
comprehensive.
