chronic disease contacts

I was recently appointed to a new local task force on chronic disease in a
large urban/ metropolitan area (Seattle/ King County). I am interested in
information on other similar task forces, especially regarding:
       1) where similar task forces exist at the local level and contacts for
them?
       2) relevant journal articles especially nutrition review articles
('91-'96) outside JADA?
       3) examples of public health core function approaches to chronic
disease            (community assessment, surveillance and monitoring,
program and policy            development, and assurance)?
