REQUEST FOR INFORMATION ON
OSTEOPOROSIS PROGRAMS
The Centers for Disease Control and Prevention (CDC)
and the U.S. Public Health Service's Office on Women's
Health (PHS OWH) in the Department of Health and
Human Services are collaborating with the National
Osteoporosis Foundation (NOF) to develop the first
national osteoporosis education campaign aimed at girls
ages 9-18.
To assist with our needs assessment, please provide
information on any osteoporosis activity within the state
that targets girls ages 9 to 18. Program information
requested includes: contact person, phone number,
e-mail address, and the following items in brief:
! name of the program
! years of the program
! target audience
! main objective(s) of the program
