Calculate Your Life Expectancy!

I know many of you have seen Websites that Calculate Your Life
Expectancy. Here's one that a colleague sent me that's interesting because
when you fill out the form and submit it, it comes back with the reasoning
behind each item in the list. It also gives you your life expectancy.

If anyone has comments on the reasoning, you might want to share them with
the authors.
