guidelines for premies

Dear Colleagues,
 I am writing to find out if you know of nutrition guidelines when working with premies in the community (after discharge from the NICU). 
We are in the process of drafting guidelines/decision trees for WIC dietitians to use in Oregon when working with premie infants. We hope to include guidance on growth grids, transitional formulas and using corrected age vs. chronological age for progression of table foods and transition of formula to cow's milk. 
Do you have guidelines that you use or know of agencies/practioners who have developed such guidelines.
