Survey research

I apologize for the crossposting...
As part of a group project in a "Survey Research Methods" course I'm taking 
(MPH program) we are to evaluate a few different surveys and devise our own 
survey.  My group has chosen nutrition beliefs, attitudes, practices and 
knowledge of health professionals. (yes, I know this is quite broad, but we 
wanted to see what type of surveys were out there!)
I was hoping someone could point me in the right direction on locating some 
sample surveys that we could review.  Actual surveys online, in journals, or 
wherever (plus any references you might have) would be extremely helpful.
