Check with nursing staff and/or patients to evaluate reading level, and
reading abilities when
they come in for dialysis. Identify the best time in the recovery cycle to
obtain the information.
Have they considered doing an automated telephone survey with the patient?

Hello
I am currently a nutrition intern at the University of Maryland Medical
Systems in Baltimore, MD. I am in the middle of my clinical management
rotation.
I am looking for a patient administered nutrition assessment tool that can
be used with kidney transplant patients who are returning to the
transplant clinic for follow up. I need a tool that can be completed by
the patient, easily scored/scanned by the nursing staff, and that will
flag any patient at nutritional risk that needs to be seen by a dietitian.
If you know of tools that are not specific to this patient population, I
would still be interested in them.
