[PHNUTR-L] Questionnaire Request

Hi everyone

I'm writing from the UK

One of my MSc students is aiming to question participants at a camp for 
overweight / obese teens about their level of nutrition knowledge / 
awareness and maybe attitudes to this.

We are looking for an appropriate (preferably validated) questionnaire 
which has been used by others with similar groups.

If anyone can point us to an appropriate instrument that would be great.

Happy to share research outcomes with a view to possible on-going 
international collaboration.
