[PHNUTR-L] Consumer sodium survey

We're looking for a standardized consumer sodium survey that we could
adapt and use within the African American population.
Our goal is to conduct a small pilot survey to assess knowledge about
recommendations for sodium intake.  We are also interested
in consumer habits, label reading and how this influences food decisions

We would appreciate your assistance in identifying surveys/tools that
would help meet our goals.
