Deadline For Applications to Extended MPH Degree Program at the University
of Washington Extended through April 14, 2000 By Request of Those Who
Responded to This Message! 

Opportunity for professionals interested in earning a MPH from an
accredited, nationally recognized program without quitting their jobs. A
summary of information about the EDP Program is below, but detailed
information and application forms can be obtained from our website at:

The Extended MPH Degree Program is a partial distance learning, partial
on-site program for professionals in or connected to public health who
wish to earn a Master's degree in Public Health through the School of
Public Health and Community Medicine at the University of Washington. Its
integrated curricula concentration is in health services management. As
defined by the Program, health services includes environmental,
educational, and personal health approaches to the solution of public
health problems. Additional pathways in health education and maternal and
child health are also available.

The onsite commitment is for one month during the summer for three years,
plus four weekend seminars per year for two years. Studies continue while
off-site by using the web and email. Most of the program's electives are
offered by distance learning only. The program is accredited, nationally
recognized, and has been offering its services to professionals for 20
years.

In addition to applying to the EDP, applicants need the following:
- A minimum of three years' experience in the health-care field
- A baccalaureat degree from an accredited college or university
- A satisfactory grade-point average (3.0) in the last graded 90
  quarter/60 semester hours

To apply to the EDP, please submit:

1) An application for the EDP program (downloadable from the web)

2) An application to the University of Washington Graduate School (on-line
   is preferrred at www.grad.washington.edu/application) and send a copy
   of the application to the program

3) Three letters of recommendation (form is downloadable from our website)

4) Graduate Record Examination (GRE) scores taken within six years
   (required of all applicants). MD's and Ph.D's may waive this
   requirement if they have taken a math course within the past 5 years
   and received a "B" or better in the course. A link to the GREs are on
   our website. GRE's may be taken by computer, but still need to be
   scheduled. GRE scores must be submitted by the deadline above.

5) Forward one official transcript from all colleges or universities
   previously attended (faxed copies can be used during the initial
   evaluation period, but originals must be received before a student can
   be admitted)
