Come learn about chronic disease self-management in Oregon -
registration is now open for the Oregon Living Well Forum! This
statewide gathering will bring together people involved with the
Stanford Chronic Disease Self-Management Program (CDSMP) and other
self-management resources for Oregonians with chronic health conditions.
 In addition to a keynote address from internationally known
self-management researcher Kate Lorig, the agenda includes an
introduction to the Living Well program, panel discussion on outreach to
diverse populations, community self-management partnerships, strategies
for sustainability, other self-management programs active here in
Oregon, and provider perspectives on self-management.  The Forum is
free; however space is limited to 100 and preference will be given to
registrants from Oregon. Mileage and lodging reimbursement are only
available to Leaders, Master Trainers, and site coordinators already
