Hi,
I am responding to the question on copyright permission forms.
I was just looking at the ALA catalog of new publications, and there is a 
book titled Copyright Law for Librarians and Educators, by Kenneth D. Crews. 
On his website there is listed a form letter for permisssions:the url is

Hope this helps, I am not an expert but thought I would see if this is the 
kind of thing you are looking for.
