The Northwest Disability Business Technical Assistance Center (NWDBTAC)
has blank postcards with check boxes that you can use to do a "report
card" on how accessible a business or governmental agency is for you.
You may complete this card, and then leave it for the business.
"Dear Business:
I recently visited __________ (Name of Business here) and noticed that
improvements could be made in the following areas to make the facility
more accessible to your customers with disabilities:
There also is a check box to let the business know "Accessibility of the
Facility is Appreciated," when you find they've done a good job of
ensuring accessibility of their facilities.
One nice thing about these cards is that the "report card" is the top half
of the card; the bottom half of the card has checkboxes for the business
to request NWDBTAC to send them information about ADA and Access
Guidelines, a Checklist for Existing Facilities, and Responsibilities as
an Employer. They simply complete the request for information, fold the
card in half, staple, then mail it to NWDBTAC.
